We cannot wait to celebrate this year's graduates with well-deserved congratulations for all their hard work! At this event, graduates will be showered with special attention that includes:
Reservations for a "Procession Time" are required to keep us organized. This is not a "sit-and-watch" event--it is highly-engaging, active family fun! Our graduation is designed to be a FREE & INTERACTIVE occasion, where families from all around Chicagoland can celebrate together (siblings too--as always!) Sign up your graduate(s), and join in the celebration! "Every child is graduating from something," so let's take the time to feature their accomplishments from this past year!
Please note: When registering, please look carefully at the different options. You will be prompted to select if you are registering your child as an individual graduate with C.I.T.Y. of Support (rainbow ribbon) or as part of your clinic's specific Graduating Class (exclusive class color). The list of participating clinics will continue to be updated, and should your clinic decide to participate after you have registered, we can always adjust your information. Check with your clinic to see if they are participating, and encourage them to visit our Sponsorship Request Form to learn ways that they and their families can get more involved!
This donation level provides 5 children with special birthday card surprises in their mailbox. The cards are created by other children in therapy as part of our "Birthday Buddies" program.
C.I.T.Y. of Support uses all donations to not only support our ongoing initiatives and events, but also to help us move forward with our vision of creating our own C.I.T.Y. Welcome Center. Thank you for participating in this celebratory fundraising opportunity!
Every graduate who registers for C.I.T.Y. of Support's Graduation Celebration will receive a professional graduation photo as part of the day's free festivities. As a token of our appreciation for your contribution to our graduation fundraiser, you will receive this customizable photo frame to commemorate this year's celebration:

This donation level allows 5 families to receive "Welcome to our C.I.T.Y." giftbags when they are discharged from the NICU at local hospitals, providing them access to support as soon as possible.

This donation level allows 5 resources to be added to a local library's "Accessible Support Collection," an extremely unique assortment of therapeutic books, tools, and toys that are not otherwise readily available for public exploration.

A separate ticket is required for each graduate being recognized. Once selected, you will be prompted to enter in your graduate's information above to personalize his/her experience!
Please indicate how many total family members/friends (non-graduates) will be attending our celebration. If registering multiple graduates, please only complete this section once (for the 1st graduate you register).
The Drive-Thru Procession will run from 12:00-12:30pm and has been designed specifically for children who may not be able to participate in the traditional, walk-thru Graduation Celebration. All children, regardless of medical complexity, sensory tolerance, and behavior are welcome to participate in our event, and this new registration option keeps these specific situations in mind.
