VOLUNTEER AGREEMENT FORM
This agreement is intended to indicate the seriousness in the relationship between RFTS and its volunteers. The intent of the Agreement is to assure volunteers of our deep appreciation of your services and to indicate our commitment to do the very best we can to make your volunteer experience a productive and rewarding one.
Reach For The Stars, Org. agrees to accept the services of _______________________(Volunteer), beginning on the date of ____________________, for the following volunteer position(s): ____________________________________.
RFTS agrees to: 1. To provide guidance, answers, and assistance to aid you in meeting your responsibilities.
2. To provide you with adequate briefs/training and feedback on your duties.
3. To respect your skills, dignity, and individual needs.
4. To be receptive to any comments or questions you may have regarding ways in which we might mutually better accomplish our respective tasks.
5. To treat you with respect, dignity and as a partner jointly responsible for completing our Mission.
I, __________________________(Volunteer name), agree to serve as a volunteer and commit to the following:
1. To perform my volunteer duties to the best of my ability.
2. To serve as a volunteer without receiving any monetary compensation or other financial benefits for my service.
3. To adhere to Reach For The Stars, Org. policies and procedures, including my own tax recordkeeping. I will respect confidentiality of agency and client information. I affirm that all information I learn about the clients (including name, address, work, personal or other affiliation) is deemed to be strictly confidential. I shall not disclose confidential information about organizational clients to any other individual or organization. I shall not take photographs of organizations' clientele without client or supervisor permission.
4. To meet time and duty commitments, or to provide adequate notice so arrangements can be made.
5. To notify Reach For The Stars Org. if my driver’s license or insurance is suspended, revoked or not renewed at expiration.
6. To provide Reach For The Stars Org. with a copy of my driver’s license and car insurance.
7. I hereby waive any claim I may now or hereafter have against Reach For The Stars, Org. for personal injury, emotional distress or property damage arising out of my services as a volunteer. I make this waiver with full understanding that many of the people Reach For The Stars, Org. serves have histories of trauma. I also agree to read and sign the attached Professional Outreach Procedures and Policies as well as the Driver Form. If applicable, I understand that I am responsible for my minor(s) and must watch them at all times while they volunteer at RFTS.
8. I understand a background check may be required. This Agreement may be terminated by either party with or without cause at any time by notice to the other party. However, I will do my best to give 2 weeks’ notice to allow RFTS to find a replacement.
