The Epworth Foundation

As a volunteer for The Epworth Foundation, I understand that my safety and others' safety are paramount. I know that driving as a volunteer is a privilege, not a right, and therefore, I agree to:

1. Provide evidence of my current status as a licensed driver 
2. Provide proof of insurance
3. Comply with all the foundation's policies and procedures and any directions provided by my supervisor/captain
4. Comply with all laws and regulations concerning driving, including laws on the use of seat belts, child safety seats, cell phone use, and speed limits
5. Promptly notify my captain of any physical conditions, vehicle defects, or road conditions that might affect my safety 
6. Notify my supervisor of any traffic citations I receive—even if given while driving on my time
7. Deliver all baskets and return undelivered baskets to the foundation

I pledge that if I drive my own vehicle on behalf of The Epworth Foundation, I will provide proof of my auto insurance. I also understand that as a volunteer driver, my personal auto insurance will be activated for any accidents or incidents involving my vehicle, including those that occur while I serve as a volunteer driver for The Epworth Foundation.

I express my understanding and intent to enter into this Driving Agreement willingly and voluntarily with my electronic signature.
